A COORDINATED SYSTEM’S PIVOT TO EXPANDED AND ENHANCED REGIONAL IMPACT

A regional coordinated homeless service delivery system faced with the unknown of COVID-19 sought Human Service Innovations support in expanding and enhancing capacity to meet the diverse and complex needs of individuals experiencing homelessness in a crisis. Challenged with multiple unknowns, the system and its partners needed an innovative and nimble response to deliver safe and sustainable solutions.

STRATEGIES AND INNOVATIONS
  1. Developed a regional systems-level strategy based on data and best practices for serving individuals experiencing homelessness in a coordinated manner.
  2. Established a triage process for segmenting resources and services based on specific population needs.
  3. Coached and advised leaders, managers and direct service practitioners in navigating process and practice modifications to best respond to the immediate crisis.
  4. dvised and provided written recommendations for structural, operational and resource allocation changes to expand and enhance the service delivery system to improve outcomes and impact beyond the crisis.
  5. Facilitated the integration and engagement of collaborative partners across public and private healthcare, mental health, social services, housing and local government leadership to optimize and expand funding resources and system capacity.
SUCCESSES
  • Effectively served 300+ additional individual/family units experiencing homelessness in the first 60 days.
  • Implemented best practice process of case conferencing to deliver a multi-disciplinary, holistic and person centered response to more effectively transition individuals to a sustainable housing solution rapidly.
  • Expanded system capacity to uniquely meet the complex needs of individuals experiencing mental health, developmental and/or intellectual disabilities.
  • Mitigated deaths from and minimized spread of COVID-19.
  • Strengthened the collaborative engagement process and structure across multiple agencies and service delivery systems to achieve greater impact in partnership with city and county leadership.
